Overseas Taiwanese Must Reside in Taiwan for Four Months Starting July to Qualify for Health Insurance

Approximately 12,000 overseas Taiwanese return to Taiwan each year to reinstate their health insurance, pay a small premium, receive medical treatment, and then immediately cancel their insurance before leaving the country.

To curb this practice, the Central Health Insurance Bureau has stipulated that starting from July 1st, Taiwanese citizens must reside in Taiwan for at least four months before they can rejoin the health insurance program.

These individuals include overseas Chinese, Taiwanese businesspeople and employees, expatriates, students studying abroad, and their dependents. It is estimated that they cost the health insurance system NT$100 million annually. The most common treatments sought are dental procedures, while the most expensive are for ischemic heart disease. They typically join the health insurance for two to three months, receive treatment, and then immediately cancel their insurance, which goes against the spirit of insurance and social fairness.

Article 16 of the Enforcement Rules of the National Health Insurance Act has been amended to require overseas Taiwanese, similar to foreign spouses, to reside in Taiwan for at least four months before they can use valid documents such as a Taiwan Area Resident Certificate, Entry and Exit Permit, Alien Resident Certificate, or Alien Permanent Resident Certificate to join the national health insurance. This is to prevent them from returning to Taiwan immediately for medical treatment when they fall ill.

Those who have been abroad for more than six months can apply to cancel their insurance at the local branch of the Health Insurance Bureau. Those who left Taiwan before July 1st of this year can be treated leniently under the old system upon their return.

For those who leave Taiwan between July 1st and December 31st, if they can provide evidence that they were unaware of the old system upon their return, they can still be treated under the old system. However, those who leave Taiwan after January 1st of next year will be subject to the new system.
