Study: Difficult Patients More Likely to be Misdiagnosed

According to a newly published study, patients who are difficult or challenge a doctor's professional competence during consultations are more likely to be misdiagnosed.
The first study involved 63 family physicians in their final year of training. They were presented with six different medical conditions, divided into two groups: ordinary patients and difficult patients. The difficult patients exhibited aggressive behavior, questioned the doctor's expertise, ignored medical advice, and did not expect support from the physician.
The doctors were asked to assess both complex and simple cases and then quickly record their diagnoses.
The results showed that in complex cases, difficult patients were 42% more likely to be misdiagnosed compared to ordinary patients. For simpler cases, the misdiagnosis rate was still 6% higher.
A second study conducted by the Institute of Medical Education Research in Rotterdam, Netherlands, involved 74 trainee physicians diagnosing eight different conditions. Half of the patients were classified as difficult to handle, while the other half were ordinary.
Researchers found that difficult patients reduced diagnostic accuracy by 20%.
Analyzing the reasons, researchers suggested that difficult patients force doctors to divert part of their attention to managing the patients' incessant demands, preventing them from focusing fully on diagnosis. Ultimately, the patients themselves suffer the consequences.
